Home Medicines Reviews

Markoste runs the HMR case from referral to claim.

The clinical work is the home visit. The problem is everything around it: uploading the GP referral, chasing the patient, booking the day, typing the report, and remembering what still needs claiming.

Each HMR sits on one board. You upload the referral, schedule the visit (with travel log), draft with Voice scribe and Markoste+, add a patient handout when needed, then complete and claim.

The problem

Most HMR time is not the consult.

Accredited pharmacists lose evenings to chase-ups, calendars, and typing. Cases stall between referral and claim when the work is split across phone notes, Word, and memory.

Referral chase

Days of follow-up

No answer, wrong number, decline, book later. Without a call log, the next attempt is guesswork.

Report write-up

60–90 min each

Medication tables, recommendations, and formatting after every visit. Clinical thinking becomes typing.

Case tracking

Ongoing mental load

Who is booked, half-written, due for re-review, or ready to claim. Spreadsheets fail under volume.

How Markoste does it

The HMR pipeline

One case moves left to right: upload referral, schedule visit with travel log, Voice scribe with Markoste+, then handout, complete, and claim.

  1. 01

    Upload the referral

You upload the GP referral to open the case. Call log then tracks every patient contact until you book or escalate.

  2. 02

    Schedule the visit

Book the home visit in PharmCal with a start time. For that day you see when to leave and the drive between stops. When you tick visits complete at home, travel km lands in your travel log.

  3. 03

    Voice scribe and Markoste+

Talk through findings after the consult. Voice scribe drafts the HMR report while Markoste+ surfaces interactions, STOPP/START flags, and deprescribing cues for you to accept or dismiss.

  4. 04

    Handout, complete, claim

Generate a patient handout from the review when you need one. Move the case to Complete, then Claimed. Monthly activity shows what you finished without a spreadsheet.

Report writing

Voice scribe drafts. Markoste+ checks.

After the consult, dictate findings in plain language. Voice scribe structures a GP-ready HMR report. Markoste+ runs on that report path so interaction and deprescribing flags appear while you edit, not in a separate tool.

  • Record in the home or dictate on the drive back
  • Markoste+ flags interactions, STOPP/START, and renal dosing cues on the draft
  • Patient handout ready when you finish the review
  • Case stays on Report Writing until you mark Complete

Around each stage

Built for the full HMR day, not just the letter.

Upload the GP referral

Start the case by uploading the referral. The board opens on Referral Received.

Call log

Log outcomes, schedule retries, and escalate to a GP letter when the patient cannot be reached.

Visit day and travel log

Book timed visits in PharmCal. Tick complete at home and estimated travel km writes to the travel log.

Voice scribe + Markoste+

Draft the report from speech while Markoste+ surfaces interactions and deprescribing flags on that draft.

Patient handout

When the review is done, generate a patient handout from the same case before you claim.

Every active HMR has a stage from referral through claimed.
