What is the ACOP program?
The Aged Care Onsite Pharmacist (ACOP) program funds pharmacists to provide regular, onsite medication management services in residential aged care. Under PPA Tier 2 arrangements, approved providers receive funding to embed pharmacy expertise in day-to-day care — not only at formal review points.
Onsite pharmacists monitor medicines use, respond to clinical issues, support quality use of medicines (QUM), and contribute evidence to the facility's governance framework. The role sits alongside — and complements — RMMR and QUM services.
Governance obligations onsite pharmacists carry
ACOP pharmacists must document interventions, maintain registers (including psychotropic medicines), participate in MAC processes, and demonstrate ongoing QUM activity. Audits such as AH-318 medication management reviews require structured evidence, not ad hoc notes.
Spreadsheets and disconnected documents break down quickly across multiple facilities. Regulators and facility managers expect traceable records — who did what, when, and what changed as a result.
How Markoste's ACOP module handles it
Markoste captures interventions from finalized clinical summaries, supports register reviews, logs facility audits, and generates quarterly governance reports exportable for MAC meetings. A multi-facility hub shows activity across your portfolio in one view.
When you use Scribe during resident reviews, structured notes feed intervention tracking automatically — reducing double entry. See the aged care pharmacist product page and ACOP guide for workflow detail.
