For Australian credentialed pharmacists
A full Home Medicines Review, without the admin grind
Watch Markoste take a case from GP referral to a report ready to claim. Most pharmacists write up in about 20 minutes per patient.
What you will see
The same path, step by step
Step 01
Upload the referral
Upload the GP referral to open the case. Patient details and documents stay on the patient file, so you are not piecing the story together from paper and email.
Step 02
Log every contact attempt and book the interview
Record each call or message in the Call log: who you spoke to, what happened, and what to do next. When the patient is ready, book the home visit on your calendar.
Step 03
Get a pre-interview checklist for each patient
Before you leave, open a checklist built for that patient: what to confirm at home, which medicines to review, and notes from the referral.
Step 04
Record the interview, with the patient's consent
With consent, record the visit. Markoste drafts the report from the conversation so you can listen and counsel. You edit the draft before anything goes to the GP.
Step 05
Markoste+ flags issues for your review
Markoste+ highlights possible issues for you to review, such as interactions and STOPP/START flags. Each flag links back to its source. You decide what belongs in the report.
Step 06
Check the claim and send it to PPA
Check the claim against the HMR Program Rules, then submit it to PPA from Markoste. Mark the case Claimed when you are done.
Markoste+ supports your clinical judgement. It does not replace it.
Every flag is for you to review, and every flag points back to its source. You decide what goes in the report.
“I cannot believe the time I saved. I completed 18 reports in one day — not because I rushed the clinical process, but because all the admin was handled for me and I could focus entirely on the clinical work.”
